PATIENTS' RIGHTS

Right to Dignity

You have the right to be treated with respect no matter what your race, color, sex, religion, age, sexual preference, beliefs, country of birth, source of payment, or kind of illness.

Right to Request

You have the right to make any reasonable request.

Right to Privacy

It is your right to expect that we will do all we can to provide for your security and protect your personal privacy. Your healthcare is considered confidential and will be discussed only with people involved in your care. You have the right to expect that all information about your care will be kept private unless you give permission, except as required by law.

Right to Identification

You have the right to know the name of any physician, nurse or other person who gives you care and the purpose of that care.

Right to Refuse

You have the right to refuse and a provider shall inform you of your right to refuse any drugs, treatment or procedure within legal limits. If you refuse, a provider will tell you the possible medical risks of the refusal.

Right to Communication

You have the right to full information, in laymen’s terms, concerning your diagnosis, treatment and prognosis, including information about benefits, alternative treatments, risks, and possible complications. When it is not possible or medically advisable to give such information to you, the information shall be given on your behalf to your designee.

If you have a hearing loss or have another special need, we will do our best to help you understand your care. If you do not speak English, you shall have access, when possible, to an interpreter. However, it is advisable to bring a friend or family member with you to your visit who can interpret for you.

Right to Knowledge

You have the right to ask questions about information contained in your medical records in a timely manner. Cabarrus Family Medicine shall provide you, or your designee, upon request, access to information contained in your medical records. A clinic representative will be present while you or your designee views your records. Under certain circumstances, your provider may restrict your access to medical records. If your provider restricts your access to information in your medical records, your provider shall record the reasons in your medical records. Access shall be restricted only for a sound medical reason. Your designee may have access to the information in your medical records even if your provider restricts your access to those records.

If requested, you should also be told the approximate cost of your treatment. You have the right to examine and receive a detailed explanation of your bill.

Right to Choose

You have a right to help make decisions about your care and to be given clear information to make those decisions. You have the right to change primary care providers if other qualified providers are available.

Cabarrus Family Medicine will care for you to the best of our ability. If you need treatment that we cannot provide, we will help you find another place that can provide the special care you need.

Resolution of Complaints

If you have a problem or conflict about your care, you have the right to talk to the person in charge or the office manager of the clinic in which you are a patient. This will in no way affect your care or treatment.
