INSURANCE & PAYMENT We participate with most major insurance plans. Since managed care plans change frequently it is important to make sure that we have correct insurance plan information. In order to use your insurance benefit, without extra out-of-pocket expense, it is your responsibility to notify us of any changes in your plan. Please present your insurance card at the time of check-in for your appointment. Fees are expected to be paid at the time of service. We accept cash, check, Visa or MasterCard. If you have an insurance copay, then you must satisfy this requirement before you leave the office. If this policy presents an undue hardship to your family now, or at any time in the future, please discuss your situation with our office manager. Payments can be made at each clinic or be mailed to: Cabarrus Family Medicine, PA P.O. Box 37908 Charlotte, NC 28237-7908 We can also accept payment through our website. Click here to pay your bill online. If you have any questions, our Patient Accounts Department is located on the second floor at 8560 Cook Street, Mt.
